If doses were missed, do not restart the series — continue from where it left off. Longer gaps do NOT reduce immunity. Maintain minimum intervals.

Golden Rules: (1) Never restart a series — continue from where it stopped. (2) A vaccine given late is still effective; missing a dose does not require restarting the entire series. (3) Never give Rotavirus after 8 months — and do not start if child is already 15 weeks or older. (4) After 7 years, use Td not DPT. (5) Minimum 5 years between Td doses.

⚠ HepB birth dose effectiveness decreases significantly after 7 days. Its purpose is perinatal transmission prevention. The complete 3-dose series (as Pentavalent at 6, 10, 14 weeks) is still required.
Rotavirus Vaccine (RVV)

⚠ TWO STRICT AGE LIMITS: (1) The FIRST dose must be given before 15 weeks of age. Do NOT start Rotavirus if the child is already 15 weeks or older. (2) ALL doses must be completed before 8 months. If child is 8 months or older, skip this vaccine entirely — do not give late under any circumstances.
Safety reason: Rotavirus vaccine carries a small risk of intussusception (bowel obstruction) in older infants. These cutoffs are WHO/IAP safety mandates. Mild diarrhoea after the dose is normal.
Pentavalent — DPT + HepB + Hib

Min interval between primary doses: 4 weeks. Min interval between last primary and Booster-1: 6 months. After age 7, whole-cell pertussis (in DPT) causes significantly more local and systemic reactions — use Td instead.

What is the catch-up schedule for missed UIP doses in India?+
Per IAP 2024: never restart a series. Pentavalent catch-up till 1 year; after that give DPT, HepB, Hib separately. Rotavirus: first dose before 15 weeks, all doses before 8 months. BCG till 1 year. MR till 5 years. JE till 15 years (endemic areas). TD anytime after 7 years with minimum 5-year interval between doses.
What is the Rotavirus vaccine age limit in India?+
TWO age limits: (1) First dose must be given BEFORE 15 weeks. (2) All doses must be completed before 8 months (32 weeks). Do not give after 8 months. If child is already 15 weeks, do not start. This is a WHO/IAP safety cutoff for intussusception risk.
What vaccines are given at birth under UIP India?+
At birth: BCG (within 24h, catch-up till 1 year), OPV-0 (at birth, up to 1 month), HepB-0 (within 24 hours, can be given up to 7 days — less effective after 7 days). Full 3-dose HepB series still required.
What is the DPT booster schedule under India UIP?+
Booster-1 at 16–24 months (catch-up till 5 years). Booster-2 at 5–6 years (catch-up till 7 years). After 7 years, use Td (not DPT). TD at 10 and 16 years with minimum 5-year interval between TD doses.
What is the minimum interval between TD doses?+
Minimum 5 years between the two TD doses (10 years and 16 years). If the 10-year dose was missed, give anytime after 7 years, then wait at least 5 years before the next TD.
What is the PCV catch-up schedule in India?+
PCV catch-up till 5 years. Starting 2–6 months: 3 doses + booster. Starting 7–11 months: 2 doses + booster. Starting 12–23 months: 2 doses, no booster. Starting ≥24 months: 1 dose. Available in Bihar, HP, MP, UP, Rajasthan, Haryana.
What is the UIP schedule at 6, 10, and 14 weeks?+
6 weeks: OPV-1, Pentavalent-1, fIPV-1, Rotavirus-1, PCV-1. 10 weeks: OPV-2, Pentavalent-2, Rotavirus-2. 14 weeks: OPV-3, Pentavalent-3, fIPV-2, Rotavirus-3, PCV-2.
What is the JE vaccine catch-up age in India?+
JE is given in endemic districts only. JE-1 at 9–12 months, JE-2 at 16–24 months. Catch-up possible up to 15 years in endemic areas. Min interval: 4 weeks.
Can vaccines be given during mild fever?+
Yes. Mild fever (<38.5°C), cold, cough, mild diarrhoea are NOT contraindications per IAP. Only severe febrile illness (>38.5°C) warrants a brief delay. Never miss a vaccination opportunity for a minor illness.
When is Pentavalent replaced by separate vaccines?+
Pentavalent (DPT+HepB+Hib combination) catch-up is only till 1 year of age. After 1 year, give DPT, HepB, and Hib as three separate vaccines. This is because the combination vaccine is not available in separate formulations for older children under UIP.
